What to Bring to the Appointment
- The patient’s insurance card
- Physician referral (only if required by insurance)
- A list of current medications the patient is taking, including dose and frequency
- The patient’s medical and dental history (see forms below), including X-rays

We accept most major insurance plans. However, you must check with your own insurance carrier regarding coverage. Also you may call our office for assistance.
